It seems if you die enough times in the same area you will receive it. If your going to play veteran trust me you will get with out trying. Good Luck.:)

Ive done like 4 missions on veteran and i guess i don't die enough cause i still don't have that achievement/trophy. ill have to just try cause almost everyone i know has that one.

You can do it on the 4th level where you have to take out the sniper. Just die 15-20 times and u will get it.

I believe you have to die in the same area 10+ times in a row before you reach the next checkpoint. I think if you go so far, die 5 times, hit a checkpoint and die 5 more, it wont count.

I got it by screwing around and yelling Bonzai! and running into a group of enemies and seeing how many I could knife before they killed me. My record was 5 :P Also, suicides do NOT count.

the last post is correct as I have a few accounts and tried to figure it out and if you hit a checkpoint then you must die (i beleive) 20 times before you should get this trophy. the best place to do it is level 4 in the house where the sniper tries to kill you but after that when they burn the building jus run in front of the second window down that hall and let the flame thrower kill you and this way there's no check point for you to worry about untill you get upstairs. or you can let the sniper kill you 20 times.
